How do I apply for services?

Start with the TeamPHCA admission process. Our intake team will help coordinate the next steps and information needed from your Supports Coordinator.

Which waivers does TeamPHCA accept?

TeamPHCA accepts PFDS, Community Living and Consolidated waivers for applicable services.

What happens before Day Program admission?

The process includes an admission form, ISP review and release, preliminary accommodation review, a Meet & Greet/Tour, authorization and welcome steps.
