TeamPHCA is developing out-of-home respite for adults 18+. The service is intended to provide temporary, planned support based on the individual's assessed needs, authorized service and person-centered plan.

TeamPHCA is planning an out-of-home respite model. Final locations, schedules and availability will be communicated before admission opens.

Staffing and support arrangements will be based on assessed needs and applicable authorization rather than a one-size-fits-all model.

AUTHORIZED

Funding matters.

Participation will depend on applicable eligibility, waiver/funding requirements, service authorization and TeamPHCA's ability to safely support the individual.

The planned TeamPHCA Respite service is for adults age 18+. Eligibility and the amount of respite available will depend on the individual's applicable waiver or funding, ISP, authorized service, assessed needs and program availability.
