We use the individual’s ISP and preferences to plan community experiences with a purpose. Support can adapt as the person practices and grows.

We coordinate with the Supports Coordinator to review ISP goals, authorized services, supervision and practical needs.
Community activities offer opportunities to practice skills, make choices and notice progress toward personal goals.
This service is designed for community settings rather than a facility-based day program. The activities selected should make sense for the individual, their goals and their approved plan.
Our intake team will help you understand the admission path and coordinate the information needed for review.

The Supports Coordinator sends the current ISP and relevant authorization information.
We discuss interests, accommodations, supervision, scheduling and transportation arrangements as applicable.
A Meet & Greet helps everyone discuss the proposed support and next steps before a start is confirmed.
TeamPHCA serves adults age 18 and older. We review each individual’s waiver, ISP, support needs and authorized services before confirming eligibility and availability.
