Some people need support at specific times. Others may need care throughout the day and night. TeamPHCA reviews the authorized plan and staffing needs before confirming a schedule or start date.
Our intake team helps coordinate the information and conversations needed to determine whether TeamPHCA can meet the requested support needs.
Share the individual’s goals, location, preferred schedule and the types of support requested.
The Supports Coordinator sends the current ISP, release and relevant authorization information.
We discuss support needs, the home setting, staffing, availability and the proposed schedule.
After the service, authorization and staffing are confirmed, we coordinate onboarding and the start of care.
TeamPHCA serves adults age 18 and older. We work with PFDS, Community Living and Consolidated Waivers for applicable authorized services.
