Day Program, 100% Community Participation Supports, Paid Family Caregiver, Employment First and 24/7 In-Home Care. Out-of-home Respite is coming soon.
TeamPHCA serves adults age 18 and older. Eligibility for each service depends on the person’s waiver, ISP, authorized support and program fit.
TeamPHCA works with PFDS, Community Living and Consolidated Waivers for applicable authorized services.
Eligible family members may be able to provide authorized support as TeamPHCA employees. The advertised rate is up to $22 per hour; actual eligibility and pay are reviewed individually.
Tell us about the individual’s goals and support needs. Our intake team can explain the options and coordinate with the Supports Coordinator.
We’ll discuss the requested service, admission information, ISP and release, then explain the appropriate review and Meet & Greet process.
